I wish to enrol my child in the following sport/s:

Basketball Yrs 1-8

Futsal Yrs 7-8

Splash Polo Yrs 3-6 (Must have a volunteer assistant coach for this sport to go ahead)

Spring Netball Yrs 3-8

Touch Yrs 1-8

Water Polo Yrs 7-8

If insufficient entries or parent volunteers are received we will be unable to enter a team.
